UnitedHealth was first out of the gate this morning (10:55 London) and it reads as a clean beat-and-raise on the headline numbers.
What they reported for Q3:
- Revenue $112.7B vs roughly $111.3–111.4B expected
- Adjusted EPS $4.69 (GAAP $4.56) vs consensus around $4.12–4.15
- Medical care ratio 88.4% vs 89.9% a year ago
- Operating cost ratio 12.8% vs 13.5% a year ago
- Full-year 2026 adjusted EPS now "at least $20.50" (GAAP at least $19.50). The July guide was $19.50–20.00.
The detail I'd keep in mind before chasing anything:
- The MCR includes $610M of favourable prior-period reserve development (2026 dates of service). Q2 had $860M of the same. So part of the cost "improvement" is reserve releases, and the call will be about how much of it is repeatable.
- For reference, the full-year MCR guide coming out of Q2 was 88.1% ± 25 bp. Any update to that range on the call matters more than the EPS beat.
- Options were pricing roughly a 6.5–7% move into the print. UNH has a history of big gaps on earnings, in both directions.
For the tape:
- Conference call is 8:00 ET / 13:00 London. Expect pre-market to whip around while management talks MCR, Medicare Advantage and Optum.
- Managed-care names (HUM, CVS, ELV, CNC) usually trade in sympathy. HUM is already in play from the Star Ratings story last week.
- Personally I'd let the first 5–15 minutes of cash set the range rather than trade the pre-market gap blind, given how thin the early book can be.

Anyone trading UNH off the open, or are you sticking to the bank prints this morning? And do you discount the reserve-release part of the beat, or does the market not care as long as the guide goes up?
